Registered Nurse (RN) – Home Health & Hospice.

Blize Healthcare | Bay Area, CA | Flexible Territory.

Blize Healthcare is seeking a compassionate and dependable Registered Nurse (RN) to join our growing home health & hospice team. In this role, you will deliver high-quality, patient-centered care in the comfort of patients’ homes and care facilities across the Bay Area.

This is an excellent opportunity for a nurse who values independence, flexibility, and meaningful patient relationships.

What You’ll Do:

Provide skilled nursing care in accordance with physician orders and care plans.
Monitor patient conditions by taking vital signs, assessing symptoms, and documenting changes.
Administer medications and treatments, including wound care.
Perform physical assessments and basic diagnostic procedures.
Collaborate with physicians and care teams to adjust treatment plans as needed.
Educate patients and families on care plans and health management.
Ensure continuity of care during after-hours and weekend coverage.

What We’re Looking For:

Active Registered Nurse (RN) license in California.
Minimum 1 year of nursing experience.
Home Health or Hospice experience strongly preferred.
Strong communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Physical ability to lift up to 50 lbs when necessary.
Valid BLS Certification.
Valid California Driver’s License and reliable transportation.

Compensation:

Start of Care (SOC): $160.
Recertification: $120.
Resumption of Care: $130.
Discharge/Death Visit: $110.
PRN Progress Visit: $80.
Hourly Rate: $40.
