Blize Healthcare is seeking a compassionate and dependable Registered Nurse (RN) to join our growing home health & hospice team. In this role, you will deliver high-quality, patient-centered care in the comfort of patients’ homes and care facilities across the Bay Area.

What You’ll Do:
  •  Provide skilled nursing care in accordance with physician orders and care plans. 
  •  Monitor patient conditions by taking vital signs, assessing symptoms, and documenting changes. 
  •  Administer medications and treatments, including wound care. 
  •  Perform physical assessments and basic diagnostic procedures. 
  •  Collaborate with physicians and care teams to adjust treatment plans as needed. 
  •  Educate patients and families on care plans and health management. 
  •  Ensure continuity of care during after-hours and weekend coverage. 

What We’re Looking For:
  •  Active Registered Nurse (RN) license in California.
  •  Minimum 1 year of nursing experience.
  • Home Health or Hospice experience strongly preferred.
  •  Strong communication and interpersonal skills.
  •  Ability to work independently and manage a flexible schedule.
  •  Physical ability to lift up to 50 lbs when necessary. 
  •  Valid BLS Certification.
  •  Valid California Driver’s License and reliable transportation. 

Compensation:
  •  Start of Care (SOC): $160.
  •  Recertification: $120.
  •  Resumption of Care: $130.
  •  Discharge/Death Visit: $110.
  •  PRN Progress Visit: $80.
  •  Hourly Rate: $40.
