About the Role:
Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator / Licensed Vocational Nurse (LVN) to join our team. This hybrid role combines office-based intake coordination with in-home patient care, ensuring a seamless transition for patients entering home health and hospice services.

This position requires both administrative work in an office setting and travel to patient homes to provide in-person nursing care.

Key Responsibilities:
Intake Coordinator Duties: (Office-Based)
  • Manage daily patient referrals and intake operations.
  • Verify insurance coverage and assist with claims processing.
  • Schedule home health and hospice visits efficiently.
  • Answer, screen, and direct phone calls.
  • Respond to patient and family inquiries in a professional and timely manner.
  • Maintain medical supply inventory and prepare DME (Durable Medical Equipment)
  • Coordinate patient admissions to ensure a smooth onboarding process.

LVN Duties: (Field-Based)
  • Travel to patient homes to provide skilled nursing care.
  • Perform basic nursing procedures (medication administration, wound care, vital signs)
  • Support patients under the direction of a Registered Nurse or physician.
  • Build compassionate relationships with patients and their families.
  • Monitor patient conditions and report changes appropriately.

Qualifications:
  • Minimum 1 year of experience in the medical field.
  • Minimum 1 year of nursing experience.
  • Home health or hospice experience preferred.
  • Strong communication, interpersonal, and customer service skills.
  • Ability to manage both administrative and clinical responsibilities.
  • Proficiency in Microsoft Office and Google Suite.

Requirements:
  • Active LVN license.
  • Valid BLS certification.
  • Valid driver’s license and reliable transportation.
  • Ability and willingness to travel to patient homes for in-home visits is required.
